Adductor-related groin pain in team sport athletes: A narrative review and practical considerations for the strength and conditioning coach

Strength and Conditioning practitioners should have a key role within the multidisciplinary team in rehabilitating and preventing adductor-related groin pain in team sport athletes, with strategies aimed towards improving hip adductor strength, multiplanar intersegmental control and linear and multidirectional running mechanics. Adductor-related groin pain has been identified as the most commonly occurring clinical entity of groin pain in team sport athletes. The aim of this review is to summarize certain risk factors associated with adductor-related groin pain, different exercise interventions outlined within the literature and the practical implications for strength and conditioning coaches. An online literature search was conducted utilizing PubMed, CINAHL, PEDro, SportDiscus and Google Scholar. A total of 31 studies relating to athletic groin pain were included within this review. Prominent risk factors identified were previous groin injury, older age, hip adductor muscle strength deficits (perhaps <80% adductor/abductor ratio as defined in hockey athletes), locomotive control, and reduced exposure to sport-specific training. Previous literature demonstrates various exercise interventions in the prevention and rehabilitation of adductor-related groin pain. Strategies aimed at improving adductor strength, multiplanar intersegmental control and linear and multidirectional running mechanics are discussed. Certain strength exercise examples, which are outlined extensively within the Practical Applications of the paper include the Copenhagen Adduction exercise, as well as specific lunge, squat, and deadlifting variations. Expert video demonstrations and explanations of targeted linear and multidirectional running drills are also included within the Practical Applications section. Strength and Conditioning practitioners have an integral role in contributing to athlete injury prevention and rehabilitation whilst operating within a multidisciplinary performance team. Collaborative interdisciplinary communication is thus a crucial element in implementing coordinated and efficacious interventions.
